Effectiveness of endometrial resection transcervical hysteroscopic

27 Background. The objectives of this study were to determine the effectiveness the 28 effectiveness of post-polypectomy hysteroscopic endometrial resection in preventing the 29 recurrence of endometrial polyps in post-menopausal patients and analyse the 30 complications and necessity of additional surgery in patients, in addition to their degree 31 of satisfaction. 32 Methods. A prospective longitudinal study of post-menopausal patients diagnosed with 33 endometrial polyps was conducted including polypectomy hysteroscopic endometrial 34 resection followed by therapeutic purposes (endometrial polyp removal) and prevention 35 of recurrence of endometrial polyps. 36 We evaluated the general condition and characteristics of the patients, including age, 37 BMI, smoking, medical and surgical history and obstetrics and menstrual status. The 38 results were analysed at several time points, six, 18, 42 and 60 months by hysteroscopy, 39 including the presence of vaginal bleeding and/or possible intracavitary pathology. 40 Results. A total of 89.5 % (n = 87) of our patients had profile factors associated with the 41 increased incidence of endometrial polyps and hyperestrogenism (diabetes mellitus, 42 hypertension and overweight); 89.5 % (n = 87) of patients were overweight; 34 % had 43 grade I obesity. The surgical procedure was safe, with a 90 % (n = 94) success rate 44 without complications, which was higher than the 84-95 % at the beginning and end 45 time points of the study. Patient acceptance and satisfaction was 90 and 84 %, 46 respectively. 47 Conclusions 48 Endometrial resection proved effective in preventing the recurrence of 49 endometrial polyps. It is a safe and effective method. Post-menopausal bleeding reduces 50 the presence of endometrial polyps. Patients reported satisfaction and acceptance of the 51 procedure. 52 53
